There are a number of forms that are used by our undergraduate Environmental Studies Certificate Program. Most of these are now
accessible on this web site. Please read through the following instructions before downloading each form, since each one differs in its
submission requirements. All forms except the course pre-registration form are in PDF format and require
Adobe Acrobat Reader to download. Paper copies of all of
these forms are also available from our office. If you prefer to pick up a paper copy, or if you have trouble accessing one of the
following forms, please drop by the Nelson Institute Academic Programs Office, Room 70 Science Hall, 550 North Park Street, Madison,
WI 53706.
You can apply to -OR- withdraw from the Environmental Studies Certificate Program by downloading and completing this form. It requires your original signature, so it should not be submitted electronically. Please complete this form and bring it to our office or mail it to us. All currently enrolled undergraduate students at UW-Madison are eligible to apply; the application process must be initiated before graduation.
This
form will ensure that we keep your Certificate file in an active status. If
you will be off campus next semester for any reason (e.g., study abroad, internship,
taking the semester off, etc), please let us know and keep us updated while
you are away. The easiest way to do that is by completing this form; you may
drop it off at our Academic Programs Office, or send it by email to Mary
Mercier.
If you would like to request transfer course equivalency through environmental studies, please download and print this form. Original signatures are required so do not submit this electronically. Instructions are included. Once this form has been completed and signed by an appropriate reviewer, it should be submitted to the Office of Transfers and Admissions in the Red Gym. Please see our page on "Credit for Study Abroad and Transfer Courses" for an explanation of how to go about obtaining course equivalencies.
If you have completed all Certificate course work (or if you are currently enrolled in the last course requirements) and if you will be graduating in the current term, please complete this Certificate/Graduation form. This information is used to post and mail completed Certificates.
If you would like to propose a course substitution, please submit the following form to the Academic Programs Office and attach a complete course syllabus. In most cases, substitutions will be considered in the Certificate's breadth categories only. Requests should be made before a course has been taken; after-the-fact substitution requests normally will not be considered except for transfer course work. Advanced Placement (AP) courses also will not be considered. Although the Certificate Program recognizes the value of pre-college AP courses, these courses cannot be used to fulfill the requirements of the Certificate, either as courses or as substitutions for courses.
